Individuals, families, professionals, and organizations can call UCP for information about cerebral palsy and other disabilities, as well as information about a variety of local, state, and national resources for persons with
disabilities of all kinds and their families. The goal of case management is to assist families in gaining access to needed medical, social, educational, and other services. To that end, case
management presently provides information and referral; pamphlets and brochures about cerebral palsy and other disabilities; resource information for adaptive equipment and toys; parent support activities, including a support
group for parents with children enrolled in the Early Childhood Intervention program; and speakers, who provide information about cerebral palsy and UCP, and other disability-related topics.
Case Management for Children age 3-20 with Disabilities is available to children who are Medicaid eligible, have complex medical issues, and live in Fort Bend County zip codes or specific Harris County zip codes (77031,
